    Job Duties and Responsibilities:-

    Work in the downtown area of Manhattan in an innovative outreach and housing placement program for adults who are street homeless.

    Responsibilities include:

    Serve as case manager for homeless adults, with a goal of placing them in transitional and permanent housing; Perform intake, assessment, service planning, advocacy, referral and escort services to assist in housing placement; Maintain client case records and enter data in computer database using handheld and desktop computers; Provide follow-up case management for clients placed in housing, to promote retention and document outcomes; Communicate with evening/weekend staff and with staff of other agencies to coordinate client service; Participate in outreach trips to identify and communicate with  homeless clients; Drive outreach van; Attend staff meetings, case conferences, training programs and supervision meetings; participate in program evaluation and improvement; Assist in maintaining program space and assisting clients who visit the office.

    Qualifications:-

    • Computer skills
    • High school diploma or GED
    • Prior experience with homelessness, mental health, and/or addictions preferred
    • Valid drivers license
    • Flexibility in work hours; available to work evening, overnight and weekend shifts when required
    • Frequent walking and standing
    • Occasional Lifting
    • Indoors and outdoors; all weather

    Schedule/Salary:-

    • Work hours are Monday to Friday 9:30am to 5:30pm but some flexibility in schedule is needed; occasional evenings and weekends are required.
    • This position is Full-Time (35 hours per week).
